2011 Youth Theatre Registration

NWTC Youth Theatre group is open to children ages 10-16. All classes are taught in English. There are a select number of spaces available, therefore selection is on a "first come, first served" basis.
Payment should have been sent before filling out this form.
This form and payment is mandatory for participation in the NWTC Youth Theatre. Past participants are also required to fill the entire form.

The NWTC would like to take and use photos from our shows in order to promote the shows and club, and to add colour to the club newsletter and to the members section of the website.
Before we go and use photos in which your child appears, we would like to get your permission.
